お申込みからご出発まで Application Procedures

• ※大学が主催する留学プログラムに参加する場合は次の項目について必ず事前にプログラム担当者へ確認してください。

• 留学プログラムによっては使用する保険が異なる場合があります。
• 留学プログラムによっては個別申込ではなく、プログラムで一括加入申込を行う場合があります。
• 個別で申込む場合、指定のプログラム名・加入プラン・保険期間にて申込を行う必要があります。

1. 1「お申込みフォーム」に必要事項をご入力の上　送信願います。※プログラムで一括加入申込の場合は不要です
2. 2「保険料払込票」をご入力頂いたご住所に郵送にてお送りいたします。
3. 3内容をご確認の上　「保険料払込票」を使用し「郵便局」で保険料をご送金願います。
4. 4メールにて「保険料払込票の領収書部分」の画像を添付の上、ご送金手続きが済まれた旨のご連絡をお願いいたします（「Kaigaku.Kyushu@tnpgrp.jp」宛にメールをお送りください）。
5. 5「被保険者証」をメールにてお送りいたします。
6. 6受け取った「被保険者証」をプログラム担当者へご提出ください。
   （提出方法についてはプログラム担当者へご確認ください）

• ※When participating in a study abroad program sponsored by a university, be sure to check the following items with the person in charge of the program in advance.

• The insurance you should use may vary depending on your study abroad program.
• Depending on the study abroad program, the person in charge of the program may apply in bulk instead of applying individually.
• If you apply individually, you need to apply with the specified program name, enrollment plan, and insurance period.

1. 1Fill in the blank of the application form.
2. 2Insurance payment form will be sent to your address.
3. 3Make a payment with the Insurance payment form at the post office within few days.
4. 4Please let us know by email that the remittance procedure has been completed with an image of the receipt part of the insurance premium payment slip.

5. 5We will send you the insurance policy by email.
6. 6Please submit the insurance policy to the person in charge of the program.
   (Please check with the person in charge of the program for the submission method)

渡航の中止や期間変更・保険の取消しや変更について Cancellation

• 渡航の中止や期間変更などによって、保険契約の取消や変更が発生する場合は、必ず弊社まで「メール」でご連絡願います。
• 保険契約の取消や変更が発生する場合は、出発迄の手続きが必要です。

• When cancelling or changing the travel, please inform us by email.
• You have to do cancellation or changing travel insurance by departure.

注意事項 Notice

• 保険料のお支払いの有無に関わらず、お申込み頂いた時点で「保険契約は成立」しています。

• ※原則、ご出発の2週間前までにお申し込みください。
• ※お申込みから被保険者（証券）到着まで約1週間程度要します。

• Whether or not you pay insurance premiums, your insurance is concluded in the time when you applied.
